Volgende pandemie is onvermijdelijk
Koopmans, sinds de uitbraak van de coronapandemie de bekendste viroloog van Nederland en internationaal al jaren een autoriteit, maakt zich grote zorgen dat de wereld niet voorbereid is op een nieuwe pandemie. De randvoorwaarden voor zo’n ramp kunnen we zó aftikken. De groeiende wereldbevolking (binnenkort passeren we de 8 miljard mensen-grens) en de meegroeiende behoefte aan dierlijke eiwitten. De intensieve veehouderij en de consumptie van bush meat. Klimaatverandering en de razendsnelle ontginning van de natuur waardoor mens en dier elkaar vaker tegenkomen en hun ziektes uitwisselen. De wereldhandel en ons reisgedrag.

Gedwongen door de stijgende temperaturen zullen veel diersoorten hun huidige habitats ontvluchten en dichter op elkaar gaan leven. Maar ook dichter bij de mens. Dat maakt het ontstaan van nieuwe ziektes, waarmee de diersoorten elkaar kunnen besmetten, maar ook de overdracht van ziekten van dier op mens gemakkelijker. “Zij zullen ook hun ziekteverwekkers delen en in het bijzonder kunnen delen met onze soort, dat wil zeggen de menselijke soort.”, zegt de bekende Franse gezondheidsecoloog Jean-François Guégan. Een internationaal team van onderzoekers voorspelt in Nature – op basis van modellen – dat er de komende 50 jaar vermoedelijk zo’n 15.000 nieuwe virusoverdrachten zullen plaatsvinden. Met het levensgrote risico dat daarbij ook een voor mensen dodelijk virus de kop zal opsteken.

Maatregelen om het risico van overspringende virussen te beperken, zouden maar $20 miljard per jaar hoeven te kosten

Potentiële pandemie-veroorzakers
Volkskrant-journalist Maarten Keulemans inventariseerde recent bij een dertigtal Nederlandse virologen, epidemiologen en infectiologen welke virussen zij kansrijk achten voor een volgende pandemie. Keulemans citeert een exotische opsomming van namen “die klinken als spreuken uit een toverboek”: chikungunya, nipah, hendra, sosugo, mayaro, uula, machupo, orungo, oropouche. Belangwekkender is de top-5 die hij presenteert van de meest waarschijnlijke kandidaten voor een volgende pandemie, waarbij overdraagbaarheid via de lucht en ‘spillover’ criteria zijn voor het ‘pandemisch potentieel’.

  1. Griep
    De dodelijkste pandemie tot nu toe was de Spaanse griep, die een eeuw geleden miljoenen mensen het leven kostte. Recent onderzoek wijst uit dat onze huidige seizoensgriep een directe afstammeling is van dat uiterst virulente H1N1-virus.
    De talloze rondwarende vogelgriepvirussen zijn een potentieel reservoir van varianten die op mensen kunnen overspringen. De gevallen waarin dat gebeurt, circuleren al langer zoals bijvoorbeeld de Russisch case van een H5N8 overdracht van kippen in een stal op zeven mensen. Griepvirussen worden pas echt eng als ze ook van mens op mens overgedragen kunnen worden.
  2. Filovirussen (ebola, Marburg)
    Een ‘meevaller’ bij deze virussen is dat de besmetting plaatsvindt via lichaamsvloeistoffen, zoals bloed, ontlasting of speeksel. Filovirusuitbraken zullen daarom – naar verwachting – regionaal beperkt blijven en uitdoven.
  3. Coronavirussen
    Alleen deze eeuw is er al drie keer een coronavirus opgedoken: SARS, MERS en SARS-CoV-2. Bij SARS lag het sterftecijfer op 10%, bij MERS (dat van dromedarissen op mensen over bleek te springen) op 35%, bij SARS-CoV-2 uiteindelijk maar op 1%. Duidelijk is inmiddels wel dat er nog honderden coronavirussen rondgaan in het wild bij uiteenlopende diersoorten.
  4. Arbovirussen (door muggen of teken overgedragen)
    Onder de verzamelnaam arbovirussen vallen virussen die bijvoorbeeld Gele koorts veroorzaken, of zika, dengue, Westnijl, Riftvalleikoorts, mayaro, of usutu. De verspreiders, muggen en teken, breiden door klimaatverandering, wereldhandel en verstedelijking hun leefgebieden steeds verder uit, al blijven de uitbraken meestal regionaal beperkt.
  5. Paramyxovirussen
    Een uitgebreide familie virussen waar ook de mazelen, bof, parainfluenza en het RS-virus toe behoren, net als nieuwkomers als nipah en hendra. Een groep waar veel virologen zich zorgen over blijken te maken, omdat er zich betrekkelijk veel nare verrassingen voordoen wanneer een paramyxovirus van dier naar mens overspringt.

Spillover
In een commentaar in Nature pleit een team onderzoekers voor een viertal maatregelen om het risico van spillovers – het overspringen van een virus van dier naar mens en dus de bron van alle trammelant – drastisch te beperken. Dat hoeft maar $20 miljard per jaar te kosten, een “kleine investering vergeleken met de miljoenen levens die verloren zijn gegaan en de biljoenen dollars die zijn uitgegeven aan de COVID-19-pandemie.” De te nemen maatregelen:

Early warning-system
Koopmans hamerde al voor de coronapandemie op de noodzaak van een plan van aanpak bij een virusuitbraak. Tevergeefs, want het lag er niet toen de coronacrisis uitbrak. Inmiddels is wel een aantal stappen gezet. Zo is er sprake van een pandemieverdrag dat het delen van virusgegevens moet stroomlijnen. Daarnaast zijn er initiatieven als Isidore (vroege signalering van nieuwe virussen bij mens en dier), Ecraid, BeyondCovid en VEO. Dit soort detectiesystemen moet eventuele nieuwkomers vroegtijdig signaleren. Liefst nog voordat ze op de mens overspringen, zodat vroegtijdig vaccins ontwikkeld kunnen worden en een nieuwe pandemie voorkomen kan worden. In plaats van de risico’s op uitbraken te beperken, vraagt onze regering de samenleving om maatregelen als het kwaad al is geschied. Optreden tegen de risico’s van spillovers vraagt de bereidheid tot internationale samenwerking en coördinatie.

Recent dook het apenpokkenvirus op buiten afgelegen natuurgebieden in Centraal- en West-Afrika. Tot nu toe zijn er 23 gevallen geconstateerd in Spanje, 5 in Portugal, 9 in het Verenigd Koninkrijk, 1 in Zweden, 1 in Italië, 1 in Frankrijk, 1 in de VS (Massachusetts) en, dichterbij huis, ook in België. In Nederland zijn op dit moment nog geen gevallen bekend. Het virus werd werd in 1958 voor het eerst het geconstateerd bij apen. Inmiddels gelden knaagdieren als de belangrijkste gastheer.

Er zijn twee stammen van het virus bekend, een ziekmakende die in 10% van de gevallen tot sterfte kan leiden en een milde en minder besmettelijke die in ongeveer 1% procent van de gevallen dodelijk kan aflopen. In Groot-Brittannië zou het gaan om een mildere variant. Volgens onder meer Marion Koopmans is er geen reden voor paniek omdat de verspreiding traag gaat, wel vindt ze het sterk verspreide optreden van de geconstateerde besmetting ‘heel ongebruikelijk’. Op gezag van een Britse viroloog meldt Reuters niettemin dat de uitbraak ‘best groot’ is en snel onder controle moet worden gebracht.


Omdat het apenpokkenvirus een pokkenvirus is, zijn virologen op hun hoede. De wereld werd in 1980 ‘pokkenvrij’ verklaard. Daarom worden bijvoorbeeld in Nederland baby’s al sinds 1974 niet meer ingeënt. De weerstand onder mensen is daardoor afgenomen. In ons land hebben alleen mensen boven de vijftig nog bescherming. Potentiële pandemie-veroorzakers
Volkskrant-journalist Maarten Keulemans inventariseerde recent bij een dertigtal Nederlandse virologen, epidemiologen en infectiologen welke virussen zij kansrijk achten voor een volgende pandemie. Keulemans citeert een exotische opsomming van namen “die klinken als spreuken uit een toverboek”: chikungunya, nipah, hendra, sosugo, mayaro, uula, machupo, orungo, oropouche. Belangwekkender is de top-5 die hij presenteert van de meest waarschijnlijke kandidaten voor een volgende pandemie, waarbij overdraagbaarheid via de lucht en ‘spillover’ criteria zijn voor het ‘pandemisch potentieel’.

  1. Griep
    De dodelijkste pandemie tot nu toe was de Spaanse griep, die een eeuw geleden miljoenen mensen het leven kostte. Recent onderzoek wijst uit dat onze huidige seizoensgriep een directe afstammeling is van dat uiterst virulente H1N1-virus.
    De talloze rondwarende vogelgriepvirussen zijn een potentieel reservoir van varianten die op mensen kunnen overspringen. De gevallen waarin dat gebeurt, circuleren al langer zoals bijvoorbeeld de Russisch case van een H5N8 overdracht van kippen in een stal op zeven mensen. Griepvirussen worden pas echt eng als ze ook van mens op mens overgedragen kunnen worden.
  2. Filovirussen (ebola, Marburg)
    Een ‘meevaller’ bij deze virussen is dat de besmetting plaatsvindt via lichaamsvloeistoffen, zoals bloed, ontlasting of speeksel. Filovirusuitbraken zullen daarom – naar verwachting – regionaal beperkt blijven en uitdoven.
  3. Coronavirussen
    Alleen deze eeuw is er al drie keer een coronavirus opgedoken: SARS, MERS en SARS-CoV-2. Bij SARS lag het sterftecijfer op 10%, bij MERS (dat van dromedarissen op mensen over bleek te springen) op 35%, bij SARS-CoV-2 uiteindelijk maar op 1%. Duidelijk is inmiddels wel dat er nog honderden coronavirussen rondgaan in het wild bij uiteenlopende diersoorten.
  4. Arbovirussen (door muggen of teken overgedragen)
    Onder de verzamelnaam arbovirussen vallen virussen die bijvoorbeeld Gele koorts veroorzaken, of zika, dengue, Westnijl, Riftvalleikoorts, mayaro, of usutu. De verspreiders, muggen en teken, breiden door klimaatverandering, wereldhandel en verstedelijking hun leefgebieden steeds verder uit, al blijven de uitbraken meestal regionaal beperkt.
  5. Paramyxovirussen
    Een uitgebreide familie virussen waar ook de mazelen, bof, parainfluenza en het RS-virus toe behoren, net als nieuwkomers als nipah en hendra. Een groep waar veel virologen zich zorgen over blijken te maken, omdat er zich betrekkelijk veel nare verrassingen voordoen wanneer een paramyxovirus van dier naar mens overspringt.

Spillover
In een commentaar in Nature pleit een team onderzoekers voor een viertal maatregelen om het risico van spillovers – het overspringen van een virus van dier naar mens en dus de bron van alle trammelant – drastisch te beperken. Koopmans hamerde al voor de coronapandemie op de noodzaak van een plan van aanpak bij een virusuitbraak. Tevergeefs, want het lag er niet toen de coronacrisis uitbrak. Dit soort detectiesystemen moet eventuele nieuwkomers vroegtijdig signaleren. Liefst nog voordat ze op de mens overspringen, zodat vroegtijdig vaccins ontwikkeld kunnen worden en een nieuwe pandemie voorkomen kan worden. In plaats van de risico’s op uitbraken te beperken, vraagt onze regering de samenleving om maatregelen als het kwaad al is geschied. Optreden tegen de risico’s van spillovers vraagt de bereidheid tot internationale samenwerking en coördinatie.

Recent dook het apenpokkenvirus op buiten afgelegen natuurgebieden in Centraal- en West-Afrika. In Groot-Brittannië zou het gaan om een mildere variant. Volgens onder meer Marion Koopmans is er geen reden voor paniek omdat de verspreiding traag gaat, wel vindt ze het sterk verspreide optreden van de geconstateerde besmetting ‘heel ongebruikelijk’. Op gezag van een Britse viroloog meldt Reuters niettemin dat de uitbraak ‘best groot’ is en snel onder controle moet worden gebracht.


Omdat het apenpokkenvirus een pokkenvirus is, zijn virologen op hun hoede. De wereld werd in 1980 ‘pokkenvrij’ verklaard.
